African Medicine Gets No Love

Although media outlets in the West typically portray Africa as a “dark continent” full of poverty, disease and war, the African continent is not only the birthplace of humankind’s civilisation sitting on some of the planet’s most astounding natural riches but has also seen and continues to see some of the world’s oldest scientific practices and discoveries and new advancements. Advancements in the form of revolutionary medical procedures such as an awake craniotomy — a surgical term for creating a temporary opening by removing part of the skull in order to carry out activity on the brain of the living being with the objective of healing an ailment or correcting an issue such as removing brain tumours.

Awake Brain Surgery

A craniotomy is brain surgery and a type of neurosurgical technique. Awake craniotomy specifically refers to performing this surgery while the patient is still fully conscious i.e. awake and not under general anaesthesia. It is done in this way to reduce the risk of potential brain damage to the patient when carrying out such a delicate operation as the neurosurgeon is able to more effectively monitor the patient's neurological state by way of live feedback and key cues as she can communicate the effects — such as the loss of a movement in a limb or the loss the ability to speak or count when the doctor manipulates a certain part of her brain. An awake craniotomy can also be applied to surgical surgeries where tissues causing epileptic seizures are removed and according to experts in the field, the technique has made removing brain tumours — whether benign or malignant, safer and much less a medical taboo than before.

Brain Surgery by Traditional Healers in Kenya

And this medical advancement is available in Kenya. Neocolonial subliminal messaging in the western media aside, perhaps one should expect such a medical feat from Kenya  —  given the existence of an old cultural medicinal practice or traditional craniotomy by the ababari emetwe (surgeons of this traditional procedure) of the Kisii or South Nyanza ethnic group within the Bantu community in Kenya dating to so far back that the earliest passing records of it are from European colonial observers in the 1950s. According to the texts contained in \"Trepanation of The Skull by The Medicine-men of Primitive Cultures, With Particular Reference to Present-day Native East African Practice\", by Edward Lambert Margetts (in Proceedings of the Third World Congress of Psychiatry, Montreal: University of Toronto Press/McGill University Press, Vol.II, 1962), trepanning in Kenyan traditional medicine, which appears to share many technical similarities with today’s awake craniotomy, has been practised by traditional healers in various East-African communities.

It was late on the first Tuesday in November, and Captain Hussen Besheir, an Ethiopian federal soldier, was on duty at a guard post outside the military camp in Dansha.

It was close to midnight when he saw headlights approaching.

Ten armed members of the Tigrayan special forces got out of the vehicle and demanded to see the camp's commander.

\"'We're not here for you',\" Hussen recalled them saying. \"'We want to talk to the leaders.'\"

Hussen refused. An argument ensued and gunfire rang out.

They were the first shots in a conflict that has since engulfed northern Ethiopia's Tigray region, killing many hundreds of people and forcing tens of thousands from their homes.

This week AFP visited the Dansha barracks, home to the Fifth Battalion of the Northern Command of the Ethiopian military, after gaining rare access to Tigray, where a near-complete communications blackout has been in place since the fighting began.

Shell casings littered the camp's grounds, and bullet holes were punched in the walls of buildings and sides of military trucks.

A metal sign at the entrance reading, \"We need to protect the constitution from anti-development forces and lead our country to renaissance,\" was so perforated with gunfire as to be almost illegible.

'Betrayal'

Hussen and others described hours-long rifle and grenade battles against fighters loyal to the Tigray People's Liberation Front (TPLF), including special forces and militiamen, joined by some federal soldiers of Tigrayan ethnicity who turned against their comrades.

Echoing a statement from Ethiopian Prime Minister Abiy Ahmed, Hussen said soldiers \"were killed in their pyjamas\", adding, \"What happened here is even worse than that.\"

\"Betrayal alone wouldn't describe the feeling that I have. These are soldiers who have been eating and drinking with us,\" he said of those former federal troops who allegedly turned their guns against them.

The government in Addis Ababa has claimed the attack on Dansha - and a simultaneous assault on another barracks in the regional capital Mekele - as justification for its military offensive in Tigray since November 4.

It points to an interview on Tigrayan media in which a prominent TPLF supporter, said a pre-emptive strike was \"imperative\".

\"Should we be waiting for them to launch attacks first? No,\" said Sekuture Getachew, in the interview, which Abiy's office has called a \"confession\".

Confrontation between Abiy and the TPLF was a long time coming. The TPLF dominated Ethiopian politics for nearly three decades until anti-government protests swept Abiy to power in 2018.

Since then the TPLF has complained of being sidelined and scapegoated for the country's woes. 

The rift widened after Ethiopia postponed national elections because of the coronavirus pandemic. Tigray went ahead with its own vote, then branded Abiy an illegitimate ruler.

Ethiopian Prime Minister Abiy Ahmed says he is launching the 'final phase' of the army's operation in Tigray with an attack on the regional capital, Mekelle. He said the military would try not to harm civilians and urged people in the city to stay at home. It comes after a deadline he gave for Tigray fighters to surrender passed on Wednesday. The TPLF party, which controls Tigray, has vowed to keep fighting. Hundreds of people have reportedly been killed and thousands have been forced from their homes. However, details of the fighting are hard to confirm because all phone, mobile and internet communications with the Tigray region have been cut. Ethiopia has so far rejected any attempts to mediate, saying the conflict is an internal matter and Mr Abiy's government is engaged in a law enforcement mission in Tigray. What did PM Abiy say? He has ordered the Ethiopian military to launch an offensive on Mekelle - a city of about 500,000 people - in the 'third and final phase' of the federal government's military campaign against the TPLF. Mr Abiy said 'great care' would be taken to protect civilians and 'all efforts' would be made to limit damage to Mekelle. He urged people in Mekelle and the surrounding areas to disarm, stay at home and away from military targets. Religious and historical sites, institutions and residential areas would not be targeted, he said. What has the TPLF said? The leader of the powerful regional party, Debretsion Gebremichael, has said Tigray forces are 'ready to die in defence of our right to administer our region'. Aid groups fear the conflict could trigger a humanitarian crisis and destabilise the Horn of Africa region. The UN has expressed alarm at the prospect of major hostilities if the Ethiopian army attacks Mekelle. African Union representatives arrived in the Ethiopian capital Addis Ababa on Thursday and are expected to meet Mr Abiy. However they will not be allowed to travel to Tigray and Ethiopia has already described offers of mediation as 'unwelcome and unlawful acts of interference'. Ethiopia's state-appointed Human Rights Commission has accused a Tigrayan youth group of being behind a massacre earlier this month in which it says more than 600 civilians were killed. The commission says the group stabbed, bludgeoned and burned to death non-Tigrayan residents of the town of Mai-Kadra with the collusion of local forces. But the TPLF has denied involvement and called for an independent international investigation. What is the fighting about? The conflict is rooted in longstanding tension between Ethiopia's central government and the TPLF, which was the dominant political force in the whole country until Mr Abiy came to power in 2018 and introduced a series of far-reaching reforms. The TPLF party, which controls Tigray, has vowed to keep fighting. Hundreds of people have reportedly been killed and thousands have been forced from their homes. However, details of the fighting are hard to confirm because all phone, mobile and internet communications with the Tigray region have been cut. Ethiopia has so far rejected any attempts to mediate, saying the conflict is an internal matter and Mr Abiy's government is engaged in a law enforcement mission in Tigray. What did PM Abiy say? He has ordered the Ethiopian military to launch an offensive on Mekelle - a city of about 500,000 people - in the 'third and final phase' of the federal government's military campaign against the TPLF. Mr Abiy said 'great care' would be taken to protect civilians and 'all efforts' would be made to limit damage to Mekelle. He urged people in Mekelle and the surrounding areas to disarm, stay at home and away from military targets. Religious and historical sites, institutions and residential areas would not be targeted, he said. What has the TPLF said? The leader of the powerful regional party, Debretsion Gebremichael, has said Tigray forces are 'ready to die in defence of our right to administer our region'. Aid groups fear the conflict could trigger a humanitarian crisis and destabilise the Horn of Africa region. The UN has expressed alarm at the prospect of major hostilities if the Ethiopian army attacks Mekelle. African Union representatives arrived in the Ethiopian capital Addis Ababa on Thursday and are expected to meet Mr Abiy. However they will not be allowed to travel to Tigray and Ethiopia has already described offers of mediation as 'unwelcome and unlawful acts of interference'. Ethiopia's state-appointed Human Rights Commission has accused a Tigrayan youth group of being behind a massacre earlier this month in which it says more than 600 civilians were killed. The commission says the group stabbed, bludgeoned and burned to death non-Tigrayan residents of the town of Mai-Kadra with the collusion of local forces. But the TPLF has denied involvement and called for an independent international investigation. What is the fighting about? The conflict is rooted in longstanding tension between Ethiopia's central government and the TPLF, which was the dominant political force in the whole country until Mr Abiy came to power in 2018 and introduced a series of far-reaching reforms. When Mr Abiy postponed a national election because of coronavirus in June, relations further deteriorated. Ethiopia’s Prime Minister Abiy Ahmed on Thursday ordered the army to launch the final offensive against dissident Tigray authorities in Mekele, the capital of the northern region.

Abiy Ahmed wrote on his facebook account saying that the army has been ordered to \"carry out the last phase\" of the operation which began on November 4 against the dissident leaders of the Tigray People's Liberation Front (TPLF).

Abiy further promised not to cause serious damage to the city and that the army will protect civilians.

This comes after the 3-day surrender ultimatum came to an end yesterday.

Abiy Ahmed had also called for non-interference from external parties as he termed this as Ethiopia's internal affair.

Thousands of people are already believed to have died and there has been widespread destruction from aerial bombardment and ground fighting since the war began on Nov. 4. Around 42,000 refugees have fled over the border to Sudan. TPLF rockets have hit neighbouring Eritrea.
